膀胱腫瘍の画像診断におけるポイント

要旨　膀胱腫瘍の質的診断，治療方針の決定において，CT，MRIの担う役割は大きい。膀胱腫瘍は，上皮性と非上皮性腫瘍に分けられる。上皮性腫瘍には移行上皮癌を始め，扁平上皮癌，腺癌，小細胞癌，絨毛癌などが含まれ，非上皮性腫瘍には平滑筋腫，褐色細胞腫，平滑筋肉腫，横紋筋肉腫，癌肉腫などが含まれる。また腫瘍と鑑別する疾患として，内膜症，炎症性偽腫瘍，他臓器からの炎症波及，などが挙げられる。CT，MRIでの石灰化，発育形態，信号強度，濃染パターンによって，病理組織の推測がある程度可能で，MRIを用いた深達度評価，CTを用いたリンパ節転移，遠隔転移の評価は治療方針の決定に有益な情報を提供する。
