要旨　近年，妊孕性や性ホルモン，美容，精神面から精巣腫瘍においても臓器温存手術が検討されるようになってきている。本術式は術中エコーモニター，迅速組織診断，および小腫瘍においては顕微鏡手術の組み合わせにより正確に行うことが可能である。また，生殖医療の進歩により，自然妊娠のほかに，患者の保存精子による妊娠の報告もされている。一方で，悪性腫瘍については根治手術が原則である。温存症例における局所再発や，少数ではあるが遠隔転移も報告されている。定期的な経過観察が必要であり，本術式の利点，欠点につき十分理解できる症例のみが適応と考える。
