要約　われわれは，ミニマム創内視鏡下手術において周術期予防的抗菌薬の段階的減量を行ってきた。現在，感染リスクの高い患者を除いて，清潔手術では抗菌薬非投与，準清潔手術では術前の単回内服投与がレジメンとなっている。その結果，泌尿器科病棟では，耐性菌検出率が著しく低下した。創が小さいこと，創縁を保護すること，創内に手指を挿入しないこと，および創部の洗浄が周術期感染予防に貢献していると思われた。
