右水腎症をきたした傍尿管下大静脈輪

文献概要

　患　者　48歳，男性。

　家族歴・既往歴　特記すべきことなし。

　現病歴　2009年7月検診の腹部超音波検査にて右水腎症を指摘され当科初診した。

　画像所見　腹部CTでは，下大静脈がL4レベルで2本に分岐し，大動脈右側を並走したのちに腎静脈下で再合流していた（図1）。右尿管は外側の下大静脈の背側より2本の下大静脈間を腹側へ通り抜けるように走行しており，より近位の尿管は拡張し水腎症を呈していた（図2）。

　臨床経過　無症状で50年近く経過しており，本人の希望にて経過観察の方針とした。
