要旨　尿路感染症は最も一般的な細菌感染症の1つで，症状の経過から急性と慢性，基礎疾患の有無により単純性と複雑性に分類される。急性単純性膀胱炎の治療は，フルオロキノロン系薬，新経口セフェム系薬が中心で，3～7日間投与する。急性腎盂腎炎の治療は軽症から中等症であれば経口薬で治療可能で，フルオロキノロン系薬，新経口セフェムを7～14日間投与する。中等症から重症例では，点滴治療（フルオロキノロン系薬，βラクタマーゼ阻害薬配合ペニシリン系薬，セファロスポリン系薬，アミノグリコシド系薬）を選択する。急性複雑性尿路感染症の治療は，耐性菌の頻度が増加するため初回治療は広域の抗菌薬を選択し，その後治療経過，薬剤感受性結果に応じて必要であれば変更を行う。
