要旨　複雑性尿路感染症は基礎疾患を有する尿路感染症であり，その成立にバイオフィルムが深く関与し，一般に難治性である。薬剤耐性化や院内感染の原因となり，臨床的に重要な病態である。基礎疾患を完全にコントロールすることが治癒への唯一の道である。よって，治療戦略として，基礎疾患への対処および適切な抗菌薬の投与が重要であるが，基礎疾患の完全な除去，および監視培養がなされていない場合の起炎菌の予測は必ずしも容易ではない。基礎疾患の改善が困難な場合には，目標を「治癒」から「細菌との共棲」，つまり治療の指標を症状の改善に置き，過剰な抗菌薬投与を避け，細菌の抗菌薬耐性化を防ぐ姿勢が重要である。
