要旨　急性精巣上体炎と急性細菌性前立腺炎は，頻度は低いものの，発熱と局所の症状により診断される泌尿器科領域の重症感染症である。共に大腸菌を始めとするグラム陰性桿菌を起炎菌としており，急性精巣上体炎では，クラミジア・トラコマティスも起炎微生物となりえる。罹患する頻度が低いため，大規模研究による確立された治療法はない。しかし，日米欧から治療の指針が示されており，これらに従って治療を進めることが望ましい。ただ，キノロン耐性大腸菌の分離頻度が高くなってきており，治療不成功となる可能性を念頭におく必要がある。
