泌尿器科領域におけるロボット手術

要旨 2009年11月,手術ロボットである「da VinciTMサージカルシステム」の製造販売が国内で初めて薬事承認された。ダビンチは鉗子類の操作性の自由度が3次元になり,術者はコンソールでの立体視による手術が施行できる。これにより,緻密な組織構築を認識することが可能となり,開腹手術より質の高い低侵襲手術が可能となった。特に,泌尿器科が専門とする後腹膜腔や小骨盤内などの狭小な空間で剝離,縫合する手術では,ダビンチの導入による手術の質の向上は明らかである。前立腺癌全摘除術を筆頭に,腎部分切除術,膀胱全摘除術,ほかにも腎盂形成術や瘻孔根治術などへの応用が見込まれる。今後,本邦での導入施設の増加が期待され,わが国も本格的なロボット手術時代の到来を迎えることとなる。
