―抗癌剤の副作用1―発熱性好中球減少症，感染症とその対策

要旨　癌化学療法に伴う発熱性好中球減少症においては，重症化や死亡のリスクは個々の患者によって異なる。リスク判定の後にすみやかに個々の患者背景に応じたempiric therapyを開始することが必要となる。本邦ガイドラインでは，初期治療として低リスク群に対しては経口ニューキノロン薬±アモキシシリン/クラブラン酸，あるいは第4世代セフェムまたはカルバペネム薬の静注による単独投与が，高リスク群に対しては第4世代セフェムまたはカルバペネム薬の静注±アミノ配糖体薬投与が推奨されている。3～5日以内に解熱した場合は，同じ薬剤でさらに数日間治療を継続，それ以外の場合は，患者背景，臨床症状や検査結果などに応じて抗菌薬の変更や追加を検討する。
