男性の尿失禁

要旨 本邦の尿失禁の頻度は,男性17.6%,女性43.9%と圧倒的に女性に多い。そして,腹圧性尿失禁の頻度は,男性2.7%,女性22.4%である。男性の腹圧性尿失禁は,頻度は低いものの難治性である。男性の腹圧性尿失禁は,良性・悪性疾患を問わず,圧倒的に前立腺摘出後,すなわち,医原性のものが多い。その他,男性の腹圧性尿失禁の原因は,放射線療法,神経因性膀胱,外傷などである。腹圧性尿失禁の標準治療は,人工括約筋埋没術である。しかし,普及していないのが問題である。その他の治療としては,膨張薬剤注入療法,スリング手術がある。切迫性尿失禁を呈する難治性排尿筋過活動の治療は,電気治療,磁気療法,ボツリヌス毒素などがあるが,未解決の問題も多い。
