精索捻転症（小児・思春期）

要旨　精索捻転症は，陰囊の急激な有痛性腫脹をきたす「急性陰囊症」の中で，診断・処置の遅れが血流障害による精巣の壊死に結びつく最も重要な救急疾患である。付属小体捻転症，精巣上体炎などとの鑑別診断が必要であり，詳細な病歴聴取，理学的所見が重要で，画像検査としては超音波カラードプラ検査が有用である。精索捻転症の疑いが少しでもあれば緊急手術の適応で，捻転の解除により血流が回復すれば精巣固定術を行い，梗塞・壊死が明らかであれば摘除する。また，健側に関しても捻転予防のために同時に固定術を行うことが一般的である。精巣温存率の向上には早期診断・治療のみならず早期受診の啓蒙も重要と考えられる。
