気腫性腎盂腎炎

要旨　気腫性腎盂腎炎は腎実質や腎周囲にガス産生像を認める重篤な壊死性尿路感染症である。糖尿病，免疫不全や尿路閉塞の患者が多く，大腸菌や肺炎桿菌が分離される。臨床症状や検査所見は他の急性腎盂腎炎と同様だが，より炎症反応が強い。診断にはCTが有用で，Wanらの分類やHuangらの分類が知られている。治療はHuangらのアルゴリズムに準じ抗菌剤単独や抗菌剤を併用しつつ経皮的ドレナージや腎摘除術を行う。予後不良因子はWanらの分類のType 1，Huangらの分類のClass 4，血小板減少，低血圧，意識障害さらに腎機能障害である。気腫性腎盂腎炎は今なお致死的であり，患者の状態によっては早急かつ積極的な治療に変更する。
