腎梗塞

要旨　腎梗塞は，腎動脈の本幹や比較的太い部位における血流の閉塞・虚血によって発症する。同じ部位の虚血でも，閉塞前（狭窄）の場合は腎動脈狭窄症と診断される（狭義の虚血性腎症）。もう少し細い腎内小動脈レベルの虚血性腎症には，コレステリン塞栓症などがある。さらに細い細動脈レベルの狭窄・閉塞によって腎硬化症が発症する。これらの疾患背景には炎症性疾患・動脈硬化・血栓症・解離性疾患などいろいろあり得るが，空間的・時間的に異なる腎動脈の狭窄や閉塞により生じる疾患の総称として，虚血性腎症（広義）と考えることができる。ここでは，腎梗塞を虚血性腎症の1疾患として説明する。
