頻尿患者から，どのようにして間質性膀胱炎を診断するか？

要旨　わが国では，間質性膀胱炎は古典的な，いわゆる間質性膀胱炎と過知覚膀胱症候群の両者を包括した疾患として取り扱われる。抗菌薬や抗コリン薬で改善しない頻尿，膀胱痛を認める場合は間質性膀胱炎を疑い，膀胱鏡検査，膀胱水圧拡張術を行うことが重要である。膀胱鏡検査では，ハンナー潰瘍の検出が診断・治療指針の決定に重要であるが，narrow band imageが簡便で正確な新しい内視鏡診断法となることが期待される。治療は，ハンナー潰瘍を認める場合は経尿道的潰瘍切除術・焼灼術を，認めない場合は診断・治療目的に膀胱水圧拡張術を行う。その後は，症状改善とQOL向上を目的に，症例ごとに適切な治療法を探していくことが必要である。
