神経疾患による過活動膀胱の治療

要旨　神経疾患に伴う過活動膀胱は，仙髄排尿中枢より上位の中枢神経障害により神経因性排尿筋過活動を呈する疾患群をさす。脳幹部橋より上位の中枢神経障害と脊髄核上型障害に分類されるが，ともに膀胱知覚障害をきたしやすいため，symptom basedによる通常の過活動膀胱診療とは異なり，治療などの評価には客観的指標が必要となる。治療法としては，排泄介助などの行動療法，抗コリン薬やA型ボツリヌス毒素膀胱壁内注入などの薬物療法，仙骨神経刺激や経皮的頸骨神経刺激療法などのneuromodulation，膀胱拡大術などの外科的治療法が挙げられるが，原因疾患による病態と患者条件により治療方法や到達目標は異なり，個々の対応が求められる。
