男性化外陰部形成術とその成績

要旨　本稿では尿道下裂および社会的性を男性とした性分化疾患に対する男性化外陰部形成術の術式と手術成績・長期予後につき概説する。非触知性腺を伴い性分化疾患が疑われる症例においては，腹腔鏡による診断が有用となる。また，固定に値する精巣を腹腔内に認める際には，腹腔鏡下の精巣固定術を行うことが選択肢となる。尿道形成術は尿道板温存の可否によりさまざまな術式があり，疾患の病態および各術式の特徴を理解したうえで術式の選択を行うことが肝要である。このような形成手術を行う際には，基礎的な知識の蓄積を重ねることは無論のこと，適切な指導者のもとで経験を積むことが必要となる。
