女性化外陰部形成術とその成績

要旨　女性化外陰部形成術の目的は，選択された性と矛盾しない外陰の形態を獲得させ，月経時の経血に支障がなく，性交渉が可能な機能的な腟を形成して，患者の精神的発達に負の影響を与えないことにある。術前には腟・子宮の発達，尿道・腟の合流部の位置を評価する必要がある。手術手技は主に陰核形成術と腟形成術からなる。陰核の短縮化には神経血管束が温存され，性的感覚が維持されるようになっている。腟形成ではtotal（partial）urogenital mobilization法により腟狭窄などの合併症が軽減されている。近年，成人期に至った患者の性機能や性活動の満足度などその長期成績も報告されるようになっている。
