当科で行っている精索静脈瘤手術の変遷

要旨　WHOは，精索静脈瘤が精巣機能の悪化や男性不妊症に明らかに関連していると結論している。当科での精索静脈瘤高位結紮術は，主に切開線，到達方法，器具の使用の変化がみられ，ルーペを用いてから精巣水瘤の発生はない。顕微鏡下精索静脈瘤低位結紮術は，局所麻酔での日帰り手術や再発の原因とされる外精静脈などの結紮ができるという利点がある。われわれは再発例の検討や術中血流方向の観察から，精管静脈と外精静脈も結紮することで良好な結果を得た。最近では精索静脈瘤の手術によって，精子のDNA fragmentation indexや異常染色体が減少，精液所見が改善し，妊娠率も改善，さらに顕微授精の妊娠率，生産率の向上，流産率の低下などが報告されている。
