095　巨大な前立腺の被膜下摘除で出血を減らしたい

Q　被膜下前立腺摘除術を開始した容量が200ml以上の巨大な前立腺肥大症の症例。出血が徐々に多くなり，輸血のことが気になってきた。


[1]概　説

　大きな前立腺肥大症に対する内視鏡治療として，ホルミウムヤグレーザー前立腺核出術（HoLEP：holmium laser enucleation of the prostate）でも十分な治療効果が上げにくいのが現状である。また，前立腺肥大症に対するグリーンライトレーザー手術（PVP）が注目されている。しかし，巨大な前立腺肥大症では手術に難渋する場合が多く，開腹による前立腺被膜下摘除術がヨーロッパやアメリカでも行われている^1～6）。
