経口ED治療薬（PDE5阻害剤）―薬剤の選択と効果

要旨　臨床の現場で使用可能なPDE5阻害剤には，シルデナフィル，バルデナフィル，タダラフィルの3剤がある。3剤の薬理学的特徴の違いをきちんと認識したうえで，使用することが望まれる。特に高齢者，肝機能障害，腎機能障害患者に対しては用法・用量などを慎重に考慮すべきである。薬歴，既往歴の確認も必要で，特に禁忌項目の除外は重要である。患者のPDE5阻害剤3剤の好みに明らかな偏りはなく，患者の自由な意思で薬剤が選択されるべきと考える。現在，on-demandな形でPDE5阻害剤は使用されているが，今後，継続的な服用，daily dosingによる，より自然な形での勃起不全の治療に期待が持たれている。
