陰茎海綿体注射療法―適応と患者管理

要旨　EDに対する血管作動薬の陰茎海綿体注射（ICI）は，PDE 5阻害剤無効あるいは禁忌症例に広く推奨されている。プロスタグランディンE₁を10～20μg/生食1ml陰茎海綿体内に注入することにより良好な勃起を得る。有効率は60～90％である。有害事象は局所の疼痛が8～24％に認められるが，重篤なものはない。勃起が4時間以上持続する持続勃起症には専門医による対処が必要である。日本ではいまだ認可に至っていないが，自己注射を含めて患者にとって極めてQOLの向上に役立つ治療法であり，今後の承認獲得に向けた活動が必要である。
