陰茎彎曲症とペロニー病の治療―保存的治療と観血的治療

要旨　先天性陰茎彎曲症とペロニー病の手術を行うのは，性交障害になる可能性の彎曲がある場合である。彎曲が軽度で陰茎長が十分であればプリケーション法，彎曲が高度な場合や短縮がある場合は，静脈移植などで海綿体を延長する。ペロニー病の場合は，発症から1年くらいは経過観察や保存的治療を行い，疼痛が激しい場合はステロイドの局注を行う。多くの場合，疼痛は改善するが彎曲や短縮は残存する。やはり性交障害などがあれば手術する。われわれの先天性陰茎彎曲症の手術法は，陰茎海綿体白膜表面を削り，2-0タイクロンでinverted sutureで行い，成積は良好である。ペロニー病の移植手術では，大伏在静脈を使用して比較的良好であるが，糖尿病患者やED合併例では移植手術は薦められない。
