求心性神経泌尿器科学 新たな概念の提案

要旨 近年,尿路知覚とそれに関連する疾患に大きな関心が集まっている1~3)。過活動膀胱3~6),間質性膀胱炎7),脊髄損傷8)の病態の背景には,膀胱求心性神経活動の亢進が関与していることが示され,膀胱求心性神経活動の亢進を抑える治療に焦点が当てられてきている。これらの尿路生殖器系の求心性神経活動と,神経活動の変化が関係する疾患を扱うにあたり,“afferent neurourology”という概念が提唱されるようになった9,10)。本稿では,この新しく提唱された“afferent neurourology”について概説し,今後の臨床応用への課題を紹介する。
