透析導入―ガイドライン，末期慢性腎不全治療の療法選択について

要旨　慢性腎臓病（chronic kidney disease：CKD）患者は増加の一途をたどっている。このような状況において，CKD患者の腎臓専門外来への受診遅延は医学的，社会的ダメージを増大させ得る。患者およびその家族へ腎代替療法〔血液透析（hemodialysis：HD）・腹膜透析（peritoneal dialysis：PD）・腎移植（renal transplantation：RTx）〕に関する情報を十分に提供することは納得した治療方法の選択につながるのみでなく，良好な患者-医師関係を保つ上でも重要であるといえる。ゆえに，ステージ4，5となったCKD患者は，可及的すみやかに一度腎臓専門外来を受診されたい。療法選択にあたり医療者に望まれることは，患者背景も十分に考慮し，短期的および長期的な目標を持ち，それぞれのニーズに合った方法を選択できるように情報提供を行うことである。
