慢性前立腺炎/慢性骨盤痛症候群の病因

要旨　慢性前立腺炎/慢性骨盤痛症候群は，前立腺もしくはその周囲に出現した炎症・組織障害が，種々の要因により慢性化し，やがて特徴的な疼痛・排尿症状・筋肉の障害・精神的な症状などを引き起こす病態である。しかし，病因など，その詳細はいまだ不明な点が多い。近年，病態に関する研究が進み，局所における炎症という概念から，全身疾患の一部とみなす概念も示唆されている。本稿では，従来検討されてきたサイトカインや免疫応答物質と炎症の関わりについて言及し，さらに近年の報告から，内分泌環境変化の関与や，症状の表現型から疾患の局在に迫るfMRI画像の有用性についても述べる。
