慢性前立腺炎/慢性骨盤痛症候群の診断

要旨　慢性前立腺炎および慢性骨盤痛症候群という名称は，1999年にNIHにて提唱された前立腺炎の分類のchronic prostatitis（CP）およびchronic pelvic pain syndrome（CPPS）を直接訳した名称である。CP/CPPS，Category Ⅲは「尿路性器の痛みをもち，一般的な方法にて尿路性器に病原性を有する細菌が検出されないもの」と分類される。さらにCategory Ⅲのうち，expressed prostatic secretions，VB3，精液に多くの白血球が観察されるものはⅢAに，上記検体に白血球が観察されないものをⅢBに分類する。診断には，症状としての「痛み」，および直腸診における前立腺の圧痛の存在，前立腺マッサージ後の検体の観察が重要である。数多くの疾患により同様な症状が起こりうるため，除外診断を十分に行う必要がある。
