腎盂・尿管腫瘍の手術

要旨　腎盂尿管腫瘍は膀胱癌と異なり，膀胱鏡や経尿道的膀胱腫瘍切除術による正確な深達度，悪性度診断が難しい腫瘍である。このため画像診断による存在診断，深達度診断の意義がいまだに大きい。この領域については，これまでの形態画像診断による。特にmultidetector computed tomography urography（MDCTU）は近年のガイドラインで推奨される第一の検査である。しかし，magnetic resonance imaging（MRI）によるurography（MRU）逆行性尿路造影などもいまだに意義のある検査である。ガイドラインによれば，腫瘍存在部位，深達度，グレードが治療方針を決定する重要な因子となる。特に，存在診断における形態画像診断ではMDCTの意義が大きい。腫瘍位置のみならず，腎杯の解剖学的形態の把握などがある。しかし，深達度診断ではMDCTによりかなり正確にはなってきているが，まだ正診率が高いとはいいがたい。グレードについては尿管鏡下生検によるものが最も正確であるが，今後はfunctional MRIなどによる機能画像診断で代替できる可能性もある。このように，腎盂尿管腫瘍における画像診断は現時点では治療方針決定のために不可欠であり，今後その意義はさらに上昇すると考えられる。
