精巣腫瘍の手術

要旨　精巣腫瘍手術における画像診断は，質・広がり・治療効果判定に必須である。原発巣に関しては，エコーでほぼ診断可能である。転移巣については，病期診断・治療効果判定にCTが用いられる。セミノーマではFDG-PETが有用な場合があり，手術適応の決定に用いられる。化学療法後の残存腫瘍切除（主に後腹膜リンパ節郭清）について，特に重要な事項は，手術前の腫瘍サイズよりも化学療法開始前の腫瘍の大きさ，範囲を基に切除範囲を決定することである。また，後腹膜以外の転移巣についても，できるだけ切除することが望ましく，化学療法前後の画像をよく吟味したうえで，切除範囲を決定する必要がある。
