055　心疾患の既往を有する患者の管理

[1]はじめに

　近年，手術方法や周術期管理の進歩に伴い，高齢者やハイリスク症例における手術症例が増加傾向にある。特に，虚血性心疾患を中心とする循環器疾患を合併している患者の割合が増加しており，われわれ泌尿器科医にとっても心血管系疾患の周術期管理が求められる。

　1996年，アメリカではACC/AHA（アメリカ心臓病学会/アメリカ心臓協会）より，非心臓手術のための周術期心血管評価ガイドラインが作成され，2002年，2007年と進歩する医療現場に合わせて改訂が行われた^1）。一方，本邦では2001年に日本循環器学会を中心に『非心臓手術における合併心疾患の評価と管理に関するガイドライン』作成班が発足した。翌年，非心臓手術における合併心疾患の評価と管理に関するガイドラインが作成され，2008年にその改訂が行われた^2）。

　手術適応を決定するうえで，泌尿器科医もこのようなガイドラインを理解しておくべきであると思われる。本項では，これらのガイドラインを参考に心疾患の既往を有する患者の管理について泌尿器科医の立場から考えてみたい。
