076　停留精巣の手術

[1]はじめに

　停留精巣は精巣が本来の下降経路（腹腔内・鼠径管・陰囊内）の途中で停留して陰囊内に降りていない状態であり，小児泌尿器科疾患の中で最も頻度の高い外科的疾患である。2005年に日本小児泌尿器科学会で『停留精巣診療ガイドライン』が作成されているので参照されたい^1）。停留精巣ほど，解剖学的特徴から診断・手術に至るまでバリエーションが多い疾患はなく，手術方法も，オーソドックスな経鼠径式精巣固定術から2006年に保険収載された腹腔鏡下精巣固定術まである。本項では，当施設で実際に行っている停留精巣の診療について紹介し，最近の知見に基づいた停留精巣の取り扱いおよび術式に応じた術前・術後管理について説明する。
