自動縫合器の選択と使い方

要旨　器械吻合に用いられる自動縫合器は，1960年代から臨床使用され始めた。改良を重ねて信頼性が向上した結果，呼吸器・消化管領域だけではなく，血管切断・肝臓・膵や尿路などの幅広い領域で使用されている。自動縫合器の基本的な仕組み，および泌尿器科手術で使用される主な機種と最近発売された製品について紹介する。泌尿器科手術において，特に体腔鏡下腎摘除術や腸管を用いた尿路変向術での腸管吻合（機能的端々吻合）に，自動縫合器が頻用されている。ほかにも，膀胱全摘，前立腺全摘，尿路再建や下大静脈腫瘍血栓を伴う腎癌に対する腎摘除術についても有用性が報告されている。これらの手術についても，機種の選択法と使用上の注意点について解説する。
