小児夜尿症診療の基礎理論

要旨　小児の夜尿症診療は，夜間多尿，過活動膀胱，睡眠覚醒閾値上昇の3要因の分析を基本に，生活指導，薬物療法，行動療法を組みあわせて行われる。国内外でガイドラインが設定されているが，夜尿症の本質的理解を得るためには，それを読むだけでは不足である。最近は，夜間睡眠時に生じる過活動膀胱という疾患概念が広まり，その本態や睡眠深度との関連についての研究もされるようになった。また，排尿蓄尿や睡眠覚醒に中心的な役割をはたし，視床とも解剖学的に連絡する脳幹部の機能がにわかに脚光を浴びている。続発性夜尿症をきたす軽症の後部尿道弁の診断と治療については，コンセンサスを確立するためのエビデンスが少しずつ集められている。
