尿管結石，疝痛発作

要旨　尿管結石の疝痛発作は極めて激しい痛みでありながらよく遭遇する病態である。鎮痛対応にはさまざまな方法があり，その中には最善とはいえない場合や，一方で経験的方法でも著効する事例がみられる。非ステロイド性消炎鎮痛剤（NSAIDs）が第一選択であることはゆるぎないが，禁忌や併存疾患により使用しにくい場合がある。また鎮痛に難渋することや，妊婦に遭遇することもある。さらに初発発作が抑えられても再発への対応も必要である。このようなことを踏まえ，意外に使いこなせていないNSAIDsの使用方法の詳細や，難治・特殊事例に対しての対応について解説する。
