良性・悪性の鑑別に迷う副腎病変

要旨　副腎腫瘍が偶然発見されるケースが増加しているが，そのうち転移も含めて悪性である可能性は少ない。しかし実際は良悪性の鑑別に苦慮するケースは少なくない。一般に腫瘍のサイズが大きくなると悪性の可能性が高くなるとされ，手術適応となることが多い。負荷試験を含む内分泌検査，CTやMRI，副腎シンチグラフィー，PET-CTなどの画像診断法を効果的に選択することが肝要であるが，良性・悪性の鑑別を早期の段階で行うことは今のところ困難である。
