良性・悪性の鑑別に迷う副腎病変

要旨　近年，CT検査やMRI検査による画像診断の進歩と普及により，副腎腫瘍の中でも偶然に発見される副腎偶発腫が増加している。副腎偶発腫は良性疾患が大半を占めるが，良性疾患であっても内分泌的異常をきたすものがあり，その病態は多様である。頻度は稀であるが，副腎皮質癌などの悪性疾患もあり，副腎偶発腫の診断においては，良性・悪性の鑑別が必要となる。一般に，腫瘍径4cm以上，内部不均一，増大傾向などの所見は悪性を示唆すると考えられているが，その鑑別は難しく，臨床所見と核医学検査を含む画像所見による総合的診断が重要である。
