腎腫瘤性病変の良性・悪性の鑑別

要旨　腎の腫瘤性病変の多くは腎細胞癌であり，良性腫瘍は比較的頻度が低い。現在，腎腫瘍は健診などの超音波検査や他疾患のCT検査時に偶然に指摘される。その多くは，3cm以下の小径腫瘤としてみつかることが多い。腫瘤は小さいものほど良性の割合が高いが，このような腫瘤は各種画像検査を駆使しても良性と悪性との鑑別が難しいことが多い。充実性腫瘍では，脂肪成分の少ない血管筋脂肪腫やオンコサイトーマを腎細胞癌と鑑別するのが難しく，画像で判断できない場合は生検も考慮される。腎囊胞性腫瘤では，囊胞性腎細胞癌と複雑性囊胞の鑑別が問題となる。いずれも，良悪の判断がつかない場合は，患者の年齢や合併症および病変の自然史を総合的に判断して対応する必要がある。
