腎盂尿管癌に対する腹腔鏡下腎尿管全摘除術

要旨　腹腔鏡下腎尿管全摘除術は，low stage症例に対する癌制御においては開腹手術とほぼ同等であることが示されている。しかし，T3以上の浸潤癌に対しての安全性はまだ示されていない。また，膀胱内再発も開腹手術よりも多いと報告されている。リンパ節郭清を行うのか，行うとすれば腹腔鏡下かそれとも開放手技かなど，腹腔鏡下手術の適応決定には，いくつかの因子を十分考慮する必要がある。また手術の際にも，腎細胞癌に対する腹腔鏡下根治的腎摘除術と異なり，尿管早期の結紮，拡張した尿管を破裂させないようにするなど，技術的な注意点がある。また，水腎症があり腎盂腎炎を起こしている症例は周囲の癒着が強く手術が容易ではない。われわれはリンパ節郭清を原則全例に行うことにしており，下部尿管癌の症例に対して腎の遊離を後腹膜鏡下に行い，下部尿管処理，膀胱カフ切除，リンパ節郭清を傍腹直筋切開により開放手技で行っている。
