特集 前立腺全摘除術後の合併症―予防と対処

要旨 欧米では，前立腺全摘除後の腹圧性尿失禁に対する外科治療として，人工尿道括約筋と尿道スリング手術が普及している。本邦でも，前者が保険収載され，重症尿失禁患者に対する治療は大きく前進した。一方，後者の軽～中等症尿失禁患者に対する治療成績は，前者に遜色なく，低侵襲かつ自然排尿を担保するため，患者数の面から考えても，早急に取り入れ，普及させるべきと思われる。尿道スリング手術には，主に，恥骨固定型と経閉鎖孔型の術式があるが，女性尿失禁治療に造詣の深い医師には，導入は必ずしも困難ではないと推測される。欧米のキットの導入，本邦における独自の工夫，両者が相まって，体系化されていくことが望まれる。
