前立腺全摘除術後の鼠径ヘルニア：危険因子と予防

要旨　前立腺全摘除術の晩期合併症として，高頻度に鼠径ヘルニア（ほとんど間接ヘルニア）を発症することが知られている。病因として，潜在的に開存していた腹膜鞘状突起が，手術に伴う腹壁の損傷により顕在化してヘルニアを発症するという仮説が有力である。危険因子として，高い年齢，痩せた症例，サブクリニカルな鼠径ヘルニアの存在，鼠径ヘルニア手術の既往，吻合部狭窄が，また術式として低侵襲手術よりも開腹手術で術後ヘルニアが多いということが報告されている。最近，術後ヘルニアを防止するさまざまな手技，工夫が考案されており，私たちは，腹膜鞘状突起を結紮，切断する手技を開発した。ほかに，精索を腹壁と腹膜から剝離する方法，内鼠径輪を縫縮する方法などが報告されている。
