113　精索静脈瘤

1　概念，病因

　男性不妊外来の30～40％の患者に精索静脈瘤が認められる。陰囊痛にて受診するケースも稀ではない。内精静脈の拡張によるうっ血により，陰囊温度の上昇，精巣内の低酸素，toxic substanceの逆流や蓄積により造精機能障害が生じる。解剖学的理由により大部分が左に生じるが，右側の造精機能も低下することより，温度上昇による精巣内酸化ストレスの亢進が考えられるが^1），骨盤内での左右の静脈のanastomosisも報告されており，その病態についてはいまだに一概には説明はできない^2）。
