各論⑤　拡大リンパ節郭清―RALPで注意すべきポイントと手術操作

要旨　ロボット支援腹腔鏡下前立腺癌摘除術（robot-assisted laparoscopic prostatectomy：RALP）は，ここ5年の間に世界的に急速な拡がりをみせ，今や米国を中心として前立腺癌手術のほぼ80％はRALPで行われるようになっている。最近では高リスクの症例に対してもRALPが適応されるようになっており，このような症例では骨盤内拡大リンパ節郭清（ePLND）も必要となってくる。da Vinciを用いることにより比較的容易に拡大郭清が可能となっており，本稿ではePLNDについての注意すべき手術操作ポイントについて述べた。
