1．膀胱癌

要旨　過活動膀胱とは，患者の検査に伴う負担を軽減し，迅速に初期治療を行えるように定義された症状であり，その原因について必ずスクリーニングを行う必要がある。膀胱癌，特に上皮内癌では，過活動膀胱に類似した蓄尿症状を認めることが多い。血尿が存在する場合には膀胱癌を疑い，膀胱鏡および尿細胞診を行うべきである。しかし，膀胱癌に関連する血尿は，間欠的にしか認めないことも多く，診療経過が長期となった場合は膀胱癌が発生する可能性があるので，いったん膀胱癌が除外されても，過活動膀胱の診療中には定期的な尿検査を継続すべきである。また，尿細胞診は特異度は高いが，感度が低い検査であることも認識しておかなければならない。
