2．尿道炎

要旨　過活動膀胱は尿意切迫感という自覚症状に基づいて診断される。そのため尿意切迫感を引き起こす下部尿路の炎症，結石など他疾患を過活動膀胱と鑑別し，それぞれ適切な治療をすることが重要である。尿道炎は鑑別すべき疾患の1つであり鑑別のポイントは尿意切迫感以外の下部尿路症状，特に排尿時痛や尿道分泌物の有無，尿検査の異常の有無である。尿道炎の鑑別は比較的容易であるが，鑑別すべき疾患として念頭に置くことは重要である。尿道の炎症がどのような機序で尿意切迫感を引き起こすのか，ラットを使った基礎研究や臨床データより排尿調節機構の1つである尿道膀胱反射が重要な役割を果たしていることを解説した。
