要旨　尿路結石症は日常臨床で遭遇する頻度が高い疾患であり，その患者数は増加し続けている。診断における画像診断には超音波検査やCTが国内外の診療ガイドラインで推奨されているが，その選択には放射線被曝量を含めた利点と欠点を考慮する。治療においては排石促進，ESWL，エンドウロロジーが3本柱となるが，長期間にわたって排石を期待し続けることはせず，積極的治療への移行を試みる。ESWLでは事前に砕石効果を予測して効果的な破砕を心がけ，合併症の回避に努める。尿路結石症は再発率が高く，加えて生活習慣病との合併も多く，メタボリックシンドロームの一疾患との概念も提唱されているので，治療後は生活習慣の改善を含めた適切な再発予防が必要である。
