陰茎癌の診断・治療

要旨　陰茎癌は稀な疾患である。稀であるがゆえに診断や治療に難渋する場合もあるが，診断は局所と鼠径リンパ節の評価が主であり，治療の根幹は両者のcontrolである。近年海外の診療ガイドラインが本邦でも普及しつつあり，それに準じた治療が行われ始めている。多くの症例は鼠径部にリンパ節を触知しないcN0症例であるが，原発巣の分化度や深達度，脈管浸潤などから鼠径リンパ節転移のリスク予測が可能となった。また進行癌症例についても，鼠径リンパ節郭清や抗癌剤治療の適応などが明確にされつつある。本稿では陰茎癌診療の現状について概説する。
