持続勃起症

要旨　持続勃起症（priapism）は日本においては比較的な稀な疾患であるため，ベテラン泌尿器科医でもその治療に困惑することが多い。虚血性持続勃起症と非虚血性持続勃起症では病態も治療も異なるため，海綿体血液ガス分析などの検査で両者を鑑別し，的確に治療することが重要である。虚血性持続勃起症は低酸素状態による組織障害のため，勃起障害になる可能性があるので，可及的すみやかに，侵襲の少ない方法から治療を始める必要がある。一方非虚血性では，初期治療は経過観察でよい。動脈塞栓術を行う場合は，治療後勃起障害などのリスクを十分患者に説明する必要がある。
