精巣捻転症

要旨　精巣捻転症（精索捻転症）は精巣に血流を供給するルートである精索が捻れて精巣が回転する病態で，新生児期，思春期に起こりやすい。多くは患側精巣の痛みと腫脹で発症し，嘔気・嘔吐を伴うこともある。理学的所見では精巣挙筋反射が参考になる。血液検査や尿検査は炎症性疾患との鑑別診断に有用である。疑われる場合は一刻も早く手術の準備に入るべきであるが，その間に可能であれば画像検査を行う。カラードプラ超音波検査が最も手軽に行え，診断価値も高い。手術のタイミングが早ければ精巣を温存可能である。治療後は，長期にわたる精液検査などでの精巣機能評価が望まれる。
