脳血管障害による神経因性膀胱（認知症を含めて）

要旨　排尿障害は，中枢神経疾患において高頻度に認められる症状の1つであり，特に脳血管障害（脳卒中，かくれ脳梗塞など），認知症（アルツハイマー病など）にしばしば合併してみられる。中枢疾患では，四肢や認知機能の障害が比較的目立つのと異なり，排尿障害は，注意深く問診をとらなければ見過ごされることも少なくない。一方，中枢疾患の初発症状として，排尿障害をきたす場合もあることから，原因不明の排尿障害をみた場合，中枢疾患を念頭に置きながら診察する必要がある。中枢障害による排尿障害は，典型的には過活動性膀胱（OAB）を呈することが多い。本稿では，中枢疾患の代表的なものである，脳卒中による神経因性膀胱について述べ，認知症についても述べる。
