二分脊椎による神経因性膀胱

要旨　「二分脊椎」の中でも，下部尿路機能障害を合併する脊髄髄膜瘤患児の神経因性膀胱に対しては，新生児期からの尿路評価と管理が必要となる。初期尿路評価はエコーおよびビデオウロダイナミクス検査を主体とするが，排尿筋外尿道括約筋協調不全などの危険因子を疑った際は，早期に予防的間欠導尿と抗コリン薬の投与を開始する。思春期になると排泄管理が自己管理に移行し，オムツやパッドの交換が面倒で間欠導尿をスキップする患児も少なくない。尿路管理の向上を図るには，患児の生活環境を考慮したうえで便失禁などの排便管理も同時に診ていく必要がある。
