低活動膀胱へのアプローチ

要旨　低活動膀胱には明確な診断基準がなく，病型分類も未確立で，治療法が限定的であるなどさまざまな問題がある。近年の研究により，低活動膀胱の原因としては，膀胱そのものの，末しょう神経，中枢神経のそれぞれの異常が複合的に関与することが示されている。また，一部の過活動膀胱は低活動膀胱に対する代償機転である可能性を示唆する報告も散見される。動物モデルを用いた低活動膀胱の病態の解明は新たな治療法開発につながる可能性が期待される。本稿では，まとまって論じられることが少ない低活動膀胱について，その疫学，原因，診断，治療とそれぞれに関する今後の課題を概説した。
